Transitional Care, also known as swing bed care, is for patients who are admitted for longer term medical treatment of an illness or rehabilitation of an injury. The program offers care to patients who require extra time to heal or become stronger before returning home. A swing bed stay at the hospital can be measured in weeks or months.
