The Front Street Lemon Creek Clinic provides comprehensive, patient-centered primary care and integrated behavioral health services for individuals and families of all ages. As a federally funded HRSA Health Center, we are committed to ensuring everyone has access to high-quality, compassionate healthcare, regardless of their ability to pay. A Sliding Fee Discount Program is available for eligible patients.

 

We welcome patients of all ages and accept most insurance plans, including Medicare, Medicaid, private insurance, and Indian Health Service (IHS) beneficiaries.

Our provider, Dr. Seth Workentine, is currently accepting new patients. Dr. Workentine is a family medicine physician with expertise in primary care and addiction medicine. He provides comprehensive care focused on prevention, wellness, chronic disease management, and personalized treatment plans to help patients achieve their best health.
