Compassionate Care That Meets You Where You Are

From time to time, everyone needs help getting through a life challenge. Whether you’re dealing with depression or just feel a little “off” but don’t understand what’s causing it, SEARHC’s Collaborative Care Program (CCP) is here for you. The program starts with a conversation between you and the healthcare provider who knows you best: your primary care provider (PCP).

Collaborative Care can help you with:

Mild to moderate depression

Anxiety

Grief

Feeling disconnected following life changes

Feeling “off”

Why Collaborative Care?

A short-term program (usually three to six months), the Collaborative Care Program is driven by a partnership between your primary care provider and a psychiatrist/behavioral health manager. Unlike traditional Behavioral Health programs which often have waiting lists, CCP begins in as little as a week. The program is not a residential program. You’ll meet with your PCP and/or behavioral healthcare manager in person or online throughout the program.

Your Collaborative Care Program team will work with you to identify your challenges, set goals, choose therapies, and track your progress. Soon enough, you’ll be ready to move forward on your own, free of the emotional load that’s been weighing you down.

Once you and your Primary Care Provider (PCP) decide that you could benefit from a Collaborative Care Program team on your side, ask your PCP to send a referral to SEARHC’s Collaborative Care Program. Learn more about obtaining a referral.
