SEARHC’s Community Health Needs Assessment is an essential tool that helps gain community insight to improve programs, expand education, and increase community resources. Lead by SEARHC’s Community Wellness team, the assessment provides useful data to identify and understand local health needs and provide a picture of population health, social determinants impact, and conditions that contribute to disparities and gaps in care delivery.

The data collected helps SEARHC understand and prioritize patient access and programming and relies on engagement and community input to ensure quality data across the community. Community members are a critical part of the feedback loop through sharing their lived experiences and helping shape health priorities that reflect local needs.

Assessment Process

Our assessment follows best practices to ensure participants’ privacy is protected. Your privacy is important, and all submissions are anonymous and will be provided in summary form during the analysis and reporting process. The assessment process includes:

Data collection and analysis of health indicators, demographics, and social determinants of health

Community engagement through surveys, listening sessions, focus groups, and collaboration with Tribal and local partners

Identification of priority health needs

4

Development of a Community Health Improvement Plan that outlines actionable strategies for the next 3–5 years

Publicly sharing results for transparency and alignment
